Payment Page
Your access to the survey questionnaire is as follows:
- You will be notified by email from the San Diego Chapter announcing the survey.
- If you are reading this, you most likely have received the notification or invitation.
- The firm administrator will be "invited" to participate in the survey with two separate email invitations. These will come from Jim Colville, the survey vendor.
- Follow the instructions in these emails and complete both the non-salary and salary questionnaires. Read below for more details.
- Keep these emails as they contain specific instructions and your unique links to the secure questionnaires.
- To receive the survey report, submit the appropriate fee. Click the payment link on this page. When the report is published you will receive an email with your unique secure link to the online PDF report.
- The non-salary and the salary (dollar amounts) are in two separate questionnaire tools.
- The non-salary questionnaire is the first link you receive. Enter the data online. You can revise your data at any time up to the survey close date. You may wish to download the paper version to assist in gathering the information.
- The salary (dollar amounts) will be submitted in a separate online tool. This tool can be accessed from the link below. You have the option of alternatively entering the salaries in an Excel file and submitting that file. See the link below. This Excel file can also be your tool to assist in gathering the salary amounts.
- The survey non-salary and salary tools can be done in any order.
- You can re-enter the online tools as many times as needed to make needed updates or revisions.

How to Participate
It's free to participate in the survey however, the survey fee must be paid in order to obtain access to the final survey report. There are price differences between participating and non-participating firms.

Benefits (non-salary) Questionnaire
This questionnaire is sent directly to you via email. Download the printable version of the survey questionnaire here. This is simply a tool to help you gather the information, it's not required. Access the online survey questionnaire from the link in the email sent directly to you. That email has more instructions. Save the email as it's the only place the link is available to you.

Salary Questionnaire
To access the salary online tool, click the link. Upon clicking the link, you will asked to enter your email address. This will be verified and you will receive an email with your unique link to gain access to the salary section of the survey. This is your only access, so keep the email easily accessable. You may enter the survey and view or edit as often as needed.

Alternate Method for Salaries Alternatively you may download the Excel Salary Questionnaire File and submit it. The online tool is very intuitive and easy to use. When downloading, if given the choice Download / Save it to your computer. Opening it online causes too many issues.

**2009 Survey Report**
The report will be available until about October 15, 2010 for those firm who have paid the report fee. Payment can be made at the Payment Link at the top of this page.
